By accepting and evaluating any Incident Investigation Form, Northwest Parkway LLC does not acknowledge or accept liability or responsibility for any alleged or claimed incident. Acceptance or receipt of the Incident Form is for review and evaluation purposes only. After receipt of the Incident Investigation Form, Northwest Parkway LLC will review and get back to you within the next 2-3 weeks, or if we need any additional information to complete our review.
